Our Members
AMH consists of licensed clinical psychologists and licensed masters level therapists. AMH provides confidential mental health services including mental health assessments and therapy for individuals, couples, families, and groups. AMH members also affiliate with community psychiatrists. Insurance Information
Most AMH providers accept Group Health and Regence Blue Shield as well as other insurance plans. Please contact your insurance company to find out what your specific benefits are. The Insurance page includes further information on insurance eligibility and questions to ask your insurance company.
Intake Forms
Prior to your first appointment, please fill out the new client forms on our Forms page. There are three forms to fill out if you are an adult seeking treatment for yourself, and five forms if you are a minor or are filling out forms for a dependent child. Please bring all completed forms to our office when you come for your first appointment. To fill out the forms you may either type your answers in the spaces provided on the web page and then print them out, or print them and then fill them in by hand. Remember to sign all printed pages where required.
Physician Referrals
Associates in Mental Health is pleased to match your patient with the appropriate therapist to provide the best services possible.
If you would like to make a general referral please have your patient or your office personnel call the AMH administration office phone number at 360-715-2488. Our Clinical Practice Coordinator will speak briefly with your patient or office personnel and will refer your patient to the therapist most qualified to meet that person's individual needs.
