Our Therapists
AMH consists of licensed clinical psychologists, and licensed masters level
therapists. AMH provides
confidential mental health services including mental health assessments
and therapy for individuals, couples, families, and groups. AMH
members

Intake Packet
An intake packet consisting of 3 forms, an Intake Sheet , Medical History Form , and Treatment Goals Form, must be filled out by all clients. Please bring all 3 completed forms into our office when you come for your appointment. Note: Due to security and confidentiality issues, we have not made it possible for you to return these forms to us electronically, however we have made it possible for you to type in your information on the forms prior to printing them.
Physician Referral Protocol
Associates in Mental Health is pleased to match your patient with the appropriate therapist to provide the best services possible. If you would like to make a general referral, please follow these steps:

Please have your patient or your office personnel call the AMH referral phone number. That number is (360) 715-2488.

Our Clinical Practice Coordinator will speak briefly with your patient or office person and will offer the services of the therapist most qualified to meet the needs of your patient.
